The Pain rate is one of the vital statistics tracked on every contestant. Pain is affected by a wide majority of threats, including but not limited to enemy attacks, weapons, traps and various other hazards.
Pain is the most common cause of death in Mazeworld for contestants and encounters alike. It can be seen as the MzW counterpart to Health Points in other role-playing games, although it works a bit differently and is not a catch-all term.
When a page mentions damage in general, it refers to the amount of Pain it deals. Other kinds of damage, and how to prevent them are discussed on the damage chart and protection chart.

- Pain decreases naturally at a rate of -2% per turn, including during fights.
- This is the only statistic that is always given explicitly to the player along with their sexual statistic, regardless of the contestant's equipment. Pain being the statistic that can be the most easily abused, it was made always explicit for balance reasons.
- A contestant should watch this stat closely, and have it as low as possible at all times.
- Received Pain can be prevented or reduced by wearing clothing of the right armor classes.
- Pain can be treated via certain medical aids, or when meeting a nurse or a medic, for a fee. The latter is a scripted event.
Effects
Pain can be felt starting from Hurt, but has no effects at this point.
If the contestant is Agonizing, his/her strength will be decreased.
Almost all creatures - whether the contestant himself/herself or encounters, will, when they reach the blackout threshold between 95 and 99% of Pain, lose consciousness. This results in this creature being unable to fight anymore. If it happens to the contestant, he or she becomes helpless and cannot do anything, instead having to wait turns until Pain subsides under 95% again. This, of course, can be very dangerous if hostiles are present in the room if it happens, as they may attack or deliver a coup de grâce to end the contestant's life.
